Summary
The aim of surgery in Non-Hodgkin gastric lymphoma is both a diagnostic and a therapeutic one. To provide an exact staging, the requirements of a staging laparotomy are to be fulfilled: exploration of the abdominal cavity, biopsies of suspected lymph nodes (D1 and D2 compartments, mesenterical, paraaortal, and iliac lymph node stations), core needle and wedge biopsies from left and right hepatic lobe, and, as indicated, from the spleen. In principle, this surgical staging can also be done by laparoscopy. If this procedure confirms a gastric lymphoma stage CS I-2 or CS II-1, the en bloc resection of the tumor by partial gastrectomy with removal of the D1 lymph node compartment is indicated. A total gastrectomy, in general, is only indicated if radicality (R0 resection) cannot otherwise be obtained. Splenectomy is not mandatory. In low-grade non-Hodgkin gastric lymphoma (CSI-1), surgery is indicated only after an unsuccessful helicobacter pylori eradication, but in high grade cases primary surgery is recommended.
Zusammenfassung
Der operative Eingriff beim Non-Hodgkin-Lymphom (NHL) des Magens (NHL) verfolgt diagnostische und therapeutische Zielsetzungen. Zur exakten Stadiendefinition sind die Anforderungen einer Staging-Laparotomie zu erfüllen: Exploration der Bauchhöhle, Biopsie suspekter Lymphknoten (Kompartment I und II des Magens bzw. mesenteriale, paraaortale und iliacale Lymphknoten), Punktionszylinder und Keil-Biopsie aus rechtem und linkem Leberlappen, gegebenenfalls auch Milz. Dieses operative Staging kann prinzipiell auch laparoskopisch durchgeführt werden. Bei hierdurch bestätigtem CS I-2 bzw. CS II-1 eines Magenlymphoms erfolgt die En-bloc-Entfernung des Tumors durch Magenteilresektion inklusive der Kompartment-I-Lymphknoten. Die Gastrektomie ist nur indiziert, wenn anderweitig keine R0-Resektion zu erzielen ist. Eine Splenektomie ist nicht obligatorischer Bestandteil des operativen Eingriffs. Im Stadium CS I-1 bei Low-Grade-NHL ist die Indikation zur Operation nach erfolgloser Heliobacter-pylori-Eradikationstherapie zu stellen, bei hochmalignen NHL allerdings bereits primär.
