Summary

Effective surgical treatment of acetabular fractures is based on high-quality standard X-rays and computed tomography (CT) scans. Classification, for example according to the system proposed by Judet/Letournel, is vital for preoperative planning. A clearly outlined concept for reduction and fixation of the individual fracture will help in the selection of the appropriate surgical approach. Among the numerous alternatives, the radical extended approaches should only be used in selected cases such as comminuted fractures of both columns or delayed surgery because of a high rate of both early and late complications. Most fractures can be treated by either the Kocher-Langenbeck or the ilioinguinal approach or by a combination of both.

Die operative Versorgung von Acetabulumfrakturen setzt aussagekräftige Röntgenaufnahmen und CT-Darstellungen voraus, worauf die Klassifikation der Verletzung z. B. nach Judet/Letournel aufbaut. Der geeignetste operative Zugang ergibt sich aus der präoperativen Planung der Reposition und Osteosynthese. Die extensiven Zugänge bieten zwar eine ausgezeichnete Übersicht, sollten aber wegen der häufigen Komplikationen nur in Ausnahmefällen (komplexe Frakturen beider Pfeiler oder verspäteter OP-Zeitpunkt) angewandt werden. Die Mehrzahl der Verletzungen läßt sich über den dorsalen Zugang nach Kocher-Langenbeck, den ventralen ilioinguinalen Zugang oder über eine Kombination beider limitierter Zugänge versorgen.
