Abstract
Clonal expansion and selection mediated by antigen stimulation has been hypothesized to contribute to the development of multiple myeloma (MM). Numerous case reports and epidemiologic surveys have linked chronic infections and inflammatory disorders to an increased incidence of MM 1,2 . Silicone implants have been associated with an increased incidence of auto-immune disorders. Much of the available information on the pathogenesis of plasma cell tumors comes from induction of peritoneal plasmacytomas by plastics and paraffin oils, such as pristane. This model does not appear to require carcinogens, transforming retroviruses or radiation. It relies upon genetic predisposition and natural mutations for the generation of the genetic changes in B cells leading to autonomous growth3. It is therefore conceivable that chronic antigen stimulation with silicone could lead to multiple myeloma in genetically predisposed individuals.
