Zusammenfassung
Beim Glaukom ohne Hochdruck, definiert als typischer glaukornatöser Gesichtsfeldausfall und typische glaukomatöse Papillenveränderung bei Augeninnen-druckwerten im statistischen Normbereich liegt vermutlich ein überwiegend augeninnendruckunabhängiger also z. B. vaskulärer Glaukomschaden vor (Abb. 1) [4, 5, 16, 39, 70-72]. Dieser überwiegend augeninnendruckunabhängige Glaukomschaden ist daher bei dieser Glaukomform an der Papille und im Gesichtsfeld isoliert quantifizierbar. Bei sekundären Glaukomen, z. B. beim Pigmentglaukom, besteht dagegen ein ausschließlich vom intraokularen Druck (IOD) abhängiger Glaukomschaden. Der augeninnendruckabhängige Glaukomschaden ist daher beim Pigmentglaukom isoliert quantifizierbar. Beim Glaucoma chronicum simplex ist der Glaukomschaden abhängig vom erhöhten intraokularen Druck [27, 42, 68] und in individuell unterschiedlichem Ausmaß von sonstigen, z. B. kardiovaskulären Risikofaktoren [14, 15, 4, 18, 22, 8, 20, 28, 75- 77,79-90] oder einem niedrigen Blutdruck. Die Analyse und Quantifizierung des weitgehend augeninnendruckunabhängigen Glaukomschadens an Papille und Gesichtsfeld beim Glaukom ohne Hochdruck im Vergleich zum augeninnen- druckabhängigen Glaukomschaden (Pigmentglaukom) und Glaucoma chronicum simplex (beide Risikofaktoren) ist von klinischem und wissenschaftlichen Interesse.
