Zusammenfassung
Implantate finden in zunehmendem Maße in vielen Bereichen der Humanmedizin Anwendung: Gelenkprothesen, das künstliche Herz, Herzklappen, Herz-schrittmacherelektroden, diverse Katheter und Shunts, Kontaktlinsen, Zahnimplantate und v.a.m. [37]. Für den Bereich der Hals-Nasen- und Ohrenheilkunde seien beispielhaft Implantate der Gehörknöchelchen oder auch des Tympanums aufgeführt. Mit zunehmenden Kenntnissen im Bereich der Medizin und der Werkstoffkunde sowie mit weiterentwikkelten Operationstechniken wie auch Techniken in der Herstellung bzw. Fertigung von Implantaten ist eine weitere Zunahme des Einsatzes von Implantaten vorauszusehen. Die einwandfreie Funktion vieler Implantate ist von ihrer Integration in das angrenzende Gewebe abhängig. Hierbei ist der Grad der Anheftung von Gewebszellen und ihre Vermehrung auf der Implantatoberfläche von entscheidender Bedeutung [18]. Die auf den atomaren Aufbau zurückzuführenden physikochemischen Eigenschaften und Wechselwirkungen der Zell- und Implantatoberflächen bestimmen die Bioverträglichkeit eines Werkstoffes. Als Konkurrenz zu Gewebszellen können Mikroorganismen die Implantatoberfläche besiedeln, wodurch beispielsweise seine Epithelisierung verhindert wird. Die Integration des Implantates wird durch eine bakterielle Besiedlung gestört und verhindert, so daß seine Funktionalität nicht gegeben ist. Der Vorgang der Besiedlung des Implantates mit körpereigenen Zellen bzw. mit Bakterien ist als Wettlauf um die Oberfläche bezeichnet worden [16].
