Management of Early Stages of Testicular Carcinoma: The Current Status

Abstract

In the last 15 years a dramatic improvement in cure rates for patients with testicular cancer from less than 50% to 90%, has been achieved by the introduction of chemotherapeutic agents such as bleomycin, ifosfamide, cisplatin, and etoposide. It has been demonstrated that patients with minimal tumor load at the time of diagnosis have an excellent survival chance, exceeding 95%. On the basis of these excellent cure rates several clinical trials were performed to investigate whether chemotherapy could be reduced in terms of number of drugs, number of cycles and dosage without affecting the cure rate. At the same time new forms of supportive treatment with third-generation antiemetics and hematopoietic growth factors have improved the tolerability of chemotherapy and reduced the risk of death owing to sepsis.
