Zusammenfassung
Dieser Beitrag beschäftigt sich mit grundlegenden Fragen der Spezifikation dessen, was ein Krankenhaus „produziert“. Er beschränkt sich dabei auf den Hauptbereich der Produktion: Die direkte Versorgung von Krankenhauspatienten. „Im Mittelpunkt der betrieblichen Betätigung im Krankenhaus steht die stationäre Vollversorgung …“ (Eichhorn 1982, S.219). Darüber hinaus werden von Krankenhäusern noch weitere Produkte erstellt. Hier sind aber nicht die selbsterstellten Vorleistungen im Rahmen der Patientenversorgung gemeint, wie beispielsweise die eigene Arzneimittelherstellung oder die eigene Wäscherei, sondern über die stationäre Patientenversorgung hinausführende Krankenhausprodukte wie die medizinische Forschung und Lehre, Ausbildung von Krankenhauspflegepersonal, Vorhaltung von Versorgungskapazitäten für Notfälle, ambulante Versorgung von Patienten und Betreiben des Rettungsdienstes.1 Diese Teile im Produktspektrum des Mehrproduktunternehmens Krankenhaus dürfen bezüglich der Versorgungsfunktion des Krankenhauses wie des daraus entstehenden Ressourcenverbrauchs nicht übersehen werden.2 Sie stehen aber nicht im Zentrum der Versorgung der eigentlichen Krankenhausfälle und werden für die Zwecke dieser Untersuchung vernachlässigt. Zum Teil können sie, wie das Beispiel einer unterschiedlichen Bewertung von Produkten in Universitätskliniken und Häusern ohne Lehraufgaben zeigt, später ohne grundsätzliche Veränderung der Produktspezifikation mit eingebaut werden. Eine Differenzierung der verschiedenen Typen von Patientenversorgung nach Fällen mit gleichen, charakteristischen Merkmalen der Versorgung wird als fallbezogene Produktspezifikation verstanden. Sie ist das Thema der weiteren Abhandlungen.
