Nachresektion beim inzidentellen Prostatakarzinom

Zusammenfassung

Das inzidentelle Karzinom ist als das vom Pathologen zufällig im transurethralen Resektionsmaterial der Prostata oder Adenomektomiepräparat entdeckte Karzinom definiert. Da der Prostatatastbefund unauffällig ist und keine klinischen Hinweise auf einen malignen Prozeß bestehen, werden diese Tumoren als Stadium 0 oder T1 (UICC 1987) von den tastbaren, klinisch manifesten Prostatakarzinomen abgegrenzt. In der Literatur finden sich zahlreiche Hinweise, daß das Inzidentalkarzinom von seiner biologischen Potenz her und bezüglich seiner Prognose kein einheitlich zu bewertender Tumor ist. Histologische Untersuchungen des Primärtumors, Staging-TUR-Befunde und prognostische Kriterien beweisen die Heterogenität dieses Tumors, der zumindest 2 Gruppen von Tumoren umfaßt, die durch die lokale Ausdehnung des Tumors und dessen Malignitätsgrad gegeneinander abgrenzbar sind. Wenn auch der Wissensstand über das unterschiedliche biologische Verhalten der inzidentellen Karzinome in den letzten Jahren erheblich erweitert wurde, so gibt es weiterhin kontroverse Ansichten über Klassifikation, biologische Bedeutung und Art des therapeutischen Vorgehens beim Inzidentalkarzinom (Kastendieck 1985).
