Zusammenfassung

Schluckstörungen sind Leitsymptom funktioneller wie organischer Erkrankungen der Speiseröhre. Der Ösophagus ist ein überwiegend muskuläres Organ, dessen Aufgabe in erster Linie im Transport der Nahrung in den Magen besteht. Die Speiseröhre wird proximal und distal von Sphinkteren eingegrenzt, die im Rahmen des Schluckaktes physiologischerweise relaxieren, damit der Speisebolus in die Speiseröhre eintreten, durch propulsiv peristaltisch ablaufende Wellen nach distal transportiert und durch schluckreflektorische Erschlaffung des unteren Sphinkters in den Magen gelangen kann. Störungen dieser komplexen Motorik führen frühzeitig zum Symptom der Dysphagie; darunter wird das Gefühl verstanden, daß ein Nahrungsbolus steckenbleibt respektive nur erschwert weitertransportiert werden kann (Berges et al. 1980; Bielefeldt u. Berges 1989a; Janisch 1986). Die Dysphagie kann mit erheblichen Schmerzen verbunden sein, die dann auch als Odynophagie bezeichnet wird.
