Zusammenfassung
Aufgrund der engen Kopplung von Funktion, Stoffwechsel und Durchblutung im zentralen Nervensystem können mittels Studien dieser physiologischen Parameter die an verschiedenen Aktivitäten beteiligten Hirnstrukturen dargestellt werden. Da Störungen der Hirnfunktion aber auch von Veränderungen in Stoffwechsel und Durchblutung gefolgt sind, pathologische Einschränkungen der Versorgung und des Energiestoffwechsels selbst auch zu Funktionseinbußen führen, sind bei vielen Erkrankungen des ZNS diese Größen meßbar verändert, ohne daß daraus Rückschlüsse auf die Ätiologie gezogen werden können. Demenzen, die vor allem als nichtlokalisierbare Störungen der Hirnleistung klinisch manifest werden, können durch konventionelle neurologische Zusatzuntersuchungen, die vor allem lokalisierte morphologische Schäden erfassen, kaum diagnostiziert werden. Bei vielen Formen sekundärer dementieller Syndrome lassen sich zwar regionale strukturelle Hirnschäden nachweisen, die Ausprägung der Demenz hängt aber oft von funktionellen Störungen primär von der Erkrankung nicht betroffener Hirnregionen ab. Die primären (degenerativen) Erkrankungen, die zur Demenz führen, sind erst in späten Stadien von im CT sichtbaren atro- phischen Veränderungen des Gehirns begleitet. Progressive Zellverluste und reduzierte Zell- und Synapsenaktivität führen zu einer Verminderung von Stoffwechsel und Durchblutung, die mittels funktioneller bildgebender Verfahren dargestellt werden können. Da die Glukose das wichtigste Substrat des Energiestoffwechsels des Gehirns ist, stellt die Untersuchung des Glukosestoffwechsels das derzeit beste Verfahren zur Erfassung und Quantifizierung funktioneller Störungen des Gehirns dar. Mittels Positronen-Emissions-Tomographie kann die Glukosestoffwechselrate regional und dreidimensional im Gehirn bestimmt werden.
