Zusammenfassung
Für die neuroradiologische Untersuchung des Felsenbeins, des Kleinhirnbrückenwinkels und der Schädelbasis stehen heute die hochauflösende Computertomographie (HACT), die Hochfeld-Kernspintomographie (HFKST) sowie die intraarterielle digitale Subtraktionsangiographie (IADSA) zur Verfügung. Im jetzigen Zeitpunkt des technologischen Entwicklungsstandes ist das Auflösungsvermögen der HACT, was die Darstellung von feinen ossären Strukturen betrifft, demjenigen der HFKST überlegen. Umgekehrt ist das Auflösungsvermögen der HFKST, was die Darstellung von Weichteilstrukturen, von Gefäßen kleineren Kalibers, von intravasalen Fluß-Phänomenen sowie des zisternalen Verlaufes von Hirnnerven betrifft, demjenigen der HACT überlegen. Aus diesen Gegebenheiten leiten sich die Indikationen für die Anwendung der HACT und der HFKST bei der Abklärung von Läsionen des Felsenbeins, des Kleinhirnbrückenwinkels und der Schädelbasis ab. Demnach stellt die HACT nach wie vor die Methode der Wahl dar für die radiologische Untersuchung des Mittelohres und des Labyrinthes, während die HFKST die Methode der Wahl für die radiologische Untersuchung des inneren Gehörganges und des Kleinhirnbrückenwinkels darstellt. Bei der Untersuchung von Läsionen des Foramen jugulare, der benachbarten Schädelbasis, des Clivus und der Pyramidenspitze kommt, im gegenwärtigen Zeitpunkt, beiden Methoden eine komplementäre Rolle zu, indem für die Erfassung der extrakraniellen Komponente der Läsion und ihrer Beziehung zu chirurgisch wichtigen ossären Strukturen (z. B. Cana-lis caroticus, Canalis n. hypoglossi, etc.) die HACT, während für die präzise Darstellung des extra- und intraduralen intrakraniellen Anteiles der Läsion und ihrer Beziehung zum Gehirn und zu wichtigen Gefäßstrukturen (z. B. A. cerebellaris anterior inferior, Sinus cavernosus, venöse Blutleiter) die HFKST zur Anwendung kommt.
