Abstract

To obtain cancer statistics, a nationwide retrospective survey of causes of death for a 3-year period was conducted in China to the 1970s. It covered 395 cities and 2136 counties with a population of more than 850 millions. Data were collected on mortality from 56 causes, including 15 types of cancers. Before this large-scale investigation a pilot survey was undertaken in 1971 in Linxian county, Henan province, which has a high incidence of cancer of the esophagus. Mobile teams of clinical and laboratory research workers were sent there by the Cancer Institute of Chinese Academy of Medical Sciences to conduct house-to-house inquiries. Meetings were held with barefoot doctors, old peasants, and grass roots leaders for the collection of information on mortality by cause. When possible, the exact cause of death was determined by examination of hospital and clinical records. Within a short time the basic epidemiologic characteristics of esophageal cancer in this county were identified and a set of methods was devised for further surveys in Anyang prefecture of Henan province, with 6 million people, and in the Tai-hang mountain area, where Henan, Hebei, and Shanxi provinces meet, including Beijing municipality, with a combined population of 50 millions. Experience showed that these methods were effective. Eventually, in 1975 they were applied in the nationwide survey of causes of death. In order to carry out this task effectively, which was conducted under the guidance of specialists, training courses were organized for local medical and health personnel and barefoot doctors. Active participation of the masses and cooperation from the Department of Health at various levels also played a crucial role.
