Zusammenfassung
Die während der vorausgegangenen 30 Jahre erzielten Fortschritte auf dem Gebiet der labormedizinischen Diagnostik von Lebererkrankungen sollen am Beispiel akuter und chronischer Hepatitiden, insbesondere der Virushepatitiden vorgestellt werden. Wesentliche Beiträge hierzu lieferten die Protein- und Enzymchemie, deren für die Leberdiagnostik bedeutende klinisch-chemische Methoden in den Jahren nach 1955 entwickelt wurden, seit dem Jahr 1965 die Immunologie und Elektronenmikroskopie. Nach der Lockerung der strengen Restriktionen für die Clonierung von Virusgenomen im Jahre 1978 gewinnt auch die Gentechnologie zunehmend an Bedeutung bei der Diagnostik viraler Hepatitiden. Auf den Grundlagen dieser Disziplinen basierende Methoden erlauben es heute, Leberschädigungen schnell und präzise zu erkennen, in vielen Fällen die Ursache der Erkrankung zu ermitteln und dadurch Hepatitiden nach ätiopathogenetischen Gesichtspunkten zu differenzieren. Hierdurch wird es möglich, spezifische therapeutische Maßnahmen einzuleiten, Aussagen über Verlauf und Prognose der Erkrankungen zu machen sowie epidemiologisch induzierte Präventivmaßnahmen, wie z. B. aktive Schutzimpfungen oder vorbeugende passive Immunisierungen, zu veranlassen.
