Zusammenfassung

Die Behandlungsergebnisse von Harnblasenkarzinomen sind nach wie vor unbefriedigend. Das hat u. E. u. a. folgende Ursachen:

  • Der endoskopisch sichtbare Tumor ist nur der umschriebene Wachstumsexzeß des sich überwiegend intra- und extramural vollziehenden Neubildungsvorganges über dessen zytogenetisches und biologisches Verhalten praetherapeutisch detaillierte Informationen fehlen.

  • Mehr als 50% der Übergangszellkarzinome haben einen sehr unterschiedlichen, schwer kalkulierbaren, mit der Tiefeninfiltration rasch zunehmenden Malignitätsgrad.

  • Ein dichtes Lymphgefäßsystem der Harnblasenwand, dessen feinkapilläre Aufzweigungen schon in der Submukosa beginnen, sorgt dafür, daß bei jeder Detrusorkontraktion sehr frühzeitig Tumorzellen über Interzellulärspalten, die die Endothelzellen trennen, schließlich in die Sammelgefäße der Adventitia einmünden.
