Zusammenfassung
Eine der Hypothesen der Pathogenese der amyotrophen Lateralsklerose (ALS) ist die Annahme eines gestörten Autoimmunmechanismus, der selektiv die Motoneurone bzw. die für die Funktion dieser Neurone bedeutsamen Zellen treffen soll (1,3,4). Die feinstrukturelle Untersuchung der neuromuskulären Übergangszone an der Endplatte hat Hinweise ergeben, daß bei ALS-Kranken bei histologisch unauffälliger postsynaptischer Region denervierte Endplatten vorliegen können. Weiterhin fanden sich im Plasma von ALS-Kranken signifikant erhöhte Aktivitäten der Azetyl-cholinesterase (AChE) (6,12). Ließe sich der in der Diagnostik der Myasthenie (15) bedeutsame Nachweis von Azetylcholinrezeptor-Antikörpern (ACh-R-AK) auch im Blut von ALS-Kranken führen, wäre dies ein weiterer Hinweis auf eine gestörte Autoimmunregulation.
