Zusammenfassung
Die akute idiopathische Polyneuritis, nach den Erstbeschreibern der typischen zytoalbuminären Dissoziation Guillain-Barré-Strohl-Syndrom (GBS) genannt, ist eine häufige Erkrankung der peripheren Nerven. Sie tritt idiopathisch ohne Vorboten oder postinfektiös nach einem meist banalen katarrhalischen oder gastrointestinalen Infekt mit einer Latenz von 2–3 Wochen akut bis subakut auf, hat nach spätestens 4 Wochen ihren Höhepunkt erreicht und klingt nach monophasischem Verlauf zumeist ohne bleibende Behinderung ab. Pathologisch-anatomisch liegt der Polyneuritis eine entzündliche segmentale Demyelinisierung mit Ödem, perivenöser Infiltration durch mononukleäre Zellen (Lymphozyten und Makrophagen) zugrunde. Etwa 3-5% der Patienten mit akutem GBS entwickeln ein oder mehrere Rezidive mit zwischenzeitlich guter Remission (1). Wegen der jeweils raschen Entwicklung im Rezidiv lassen sich diese Formen dem akuten GBS zuordnen und von den chronisch progredienten bzw. chronisch rezidivierenden idiopathischen Polyneuritiden (CRIP) abgrenzen.
