Zusammenfassung
Die Radiologie am Krankenbett spielt in der Intensivmedizin eine besondere Rolle in der Diagnostik und Verlaufsbeobachtung, insbesondere auch deswegen, weil eine klinische Untersuchung bei beatmeten Patienten mit Herzkreislaufmonitoring sehr schwierig ist (Henschke et al. 1983). Der Radiologe muß nicht nur das breite Spektrum der Ursachen und des Verlaufes der kardio-respiratorischen Komplikationen kennen, sondern auch die technischen Faktoren und die Effekte der apparativen Beatmung berücksichtigen. Besonders wichtig ist die radiologische Lagekontrolle von Kathetern, Trachealtuben und Drains, aber auch die Durchführung spezieller Untersuchungen, wie z.B. die Pulmonalisangiographie (BOPA) und die Sonographie (Abb. 1). Bettseitige Röntgenuntersuchungen sind ein wichtiger Teil im multidisziplinären Management in der Intensivstation. Die Röntgenologie am Intensivbett hat sich zu einem speziellen Teilbereich entwickelt (Pokieser 1981). Die Befundung der Untersuchungen sollte gemeinsam mit den Intensivärzten erfolgen, damit der Radiologe eine möglichst umfassende klinische Information über den Patienten erhält und ein rascher Informationsfluß gewährleistet ist. Die Bedeutung der Röntgendiagnostik auf einer Intensivstation geht daraus hervor, daß in 40–70% der Lungenröntgenbilder wesentliche, bis dahin noch nicht bekannte Befunde erhoben werden (Janower et al. 1984; Henschke et al. 1983).
