Zusammenfassung
Die Prognose des Prostatakarzinoms ist in erster Linie abhängig vom Stadium und dem Malignitätsgrad. Mit allen Behandlungsmethoden weisen undifferenzierte Prostatakarzinome eine schlechtere Prognose auf, auch histologisch ist der Regressionsgrad nach Hormon- und/oder Strahlentherapie bei undifferenzierten Prostatakarzinomen geringer als bei differenzierten [1, 3, 8], ebenfalls klinisch die lokale Tumorkontrolle. Die Frequenz persistierender Tumorzellen 1–2 Jahre nach der Therapie ist abhängig von der Tumorgröße, also von dem mit der Tumorgröße ansteigenden Anteil undifferenzierter Tumorzellen [10] sowie von der Dosis; allerdings ergibt sich kein entscheidender Einfluß auf die Rezidivrate in T4 mit Dosen oberhalb 65 Gy [2, 6], sondern ein Anstieg der Komplikationsrate. Patienten mit positiver Biopsie 1–2 Jahre nach der Megavolttherapie weisen eine schlechtere Prognose auf [2, 7] als mit negativer Biopsie.
