Lymphknotendissektion beim Seminom im Stadium I, IIa und IIb?

Zusammenfassung

Ausgehend von der klinischen Beobachtung einer Tumorprogression nach Radiatio von Lymphknotenmetastasen eines Seminoms im Stadium IIa, wurde auf der 30. Jahrestagung der Deutschen Gesellschaft für Urologie 1978 das Postulat erhoben, bei Patienten der Stadien I, IIa und IIb eine modifizierte bzw. radikale Lymphknotendissektion durchzuführen. Theoretischer Hintergrund dieser Forderung stellte das mögliche Übersehen eines zweiten nicht seminomatösen Tumoranteils in der Histologie dar, insbesondere bei ausgeprägten Hodentumormassen. Außerdem ist die Differentialdiagnose zwischen anaplastischem bzw. reinem Seminom mit starker lymphozytärer Infiltration auf der einen Seite und solid wachsendem Embrionalcarcinom auf der anderen Seite schwer zu fällen [1, 2, 3]. Eine weitere Argumentation liegt in dem sog. Burned Out Phänomen, d.h. dem Zugrundegehen eines nicht seminomatösen Tumoranteils im Hoden, der vorher jedoch schon Metastasen gesetzt hat.
