Abstract
Recently several major events (e.g., Three Mile Island, the Challenger Shuttle accident, Chernobyl) have shocked man’s consciousness as to the possible consequences that advanced technology could impart. Unfortunately, some interpretations of these events as technological failures (accidents) have masked the important role that information plays in such occurrences. On a more positive note however, these occurrences provide the basis for studies that are directed specifically to the significant role that information plays in such events.
This paper was written to provide guidance to those individuals who prepared working papers concerned with systemic issues.
