Zusammenfassung

Die Anwendung stark wirksamer Diuretika (Typ Schleifendiuretika) bei fortgeschrittenen Lebercirrhosen mit Ascites ist risikobeladen. Das resultiert aus dem Sachverhalt, daß mit zunehmender Schwere der Leberinsuffizienz die Nierendurchblutung abnimmt [1]. Schleifendiuretika, die erfahrungsgemäß bei therapieresistentem Ascites eingesetzt werden, können durch Volumendepletion aber auch infolge der Blockierung des tubulären Harnsäuretransports eine drastische Verschlechterung der Nierenfunktion mit Oligo-Anurie und Elektrolytverschiebungen auslösen. Sie erhöhen damit die Gefahr, daß sich eine bereits bestehende funktionelle Niereninsuffizienz (hepatorenales Syndrom) durch Therapiemaßnahmen verschlechtert oder manifest wird. Auch die kombinierte An-wendung von Spironolacton und Schleifendiuretika bei gleichzeitiger Natriumrestriktion der Kost führte in 26% von 86 Patienten eine signifikante Verschlechterung der Nierenfunktion herbei [2].
