The Identification Block

Abstract

To identify data collected from an individual patient at a single examination and to prevent loss of such data, each of the separate AGP assessment sheets has the same basic information block (ID) printed at the top of the sheet. Complete and accurate encoding of the ID block is necessary for the collection of material and the subsequent data processing and analyses. Since experience has shown that errors of omission and commission within the ID block are a major problem in data editing, raters are urged to be particularly conscientious in completing this block.
