Zusammenfassung

Unter nekrotisierenden Arteriitiden faßt man eine Reihe von klinischen Syndromen zusammen, die durch ein ähnliches pathologisches Substrat und möglicherweise auch durch eine ähnliche immunologische Basis gekennzeichnet sind. Die histopatho- logischen Kriterien entsprechen weitgehend denen der kutanen nekrotisierenden Venulitis bzw. leukozytoklastischen Angiitis. Es spielen sich diese Veränderungen aber in einer anderen, nämlich tieferen Etage des kutanen Gefäßplexus ab, wobei nicht nur die mittelgroßen bis größeren muskulären Arterien der Haut und Subkutis sondern auch die der Muskulatur und der inneren Organe in beliebiger Kombination befallen sein können. Zusätzlich zu dem den meisten nekrotisierenden Vaskulitiden zugrunde liegenden Mechanismus der intramuralen Deposition von großen löslichen zirkulierenden Immunkomplexen mit anschließender Komplementaktivierung und neutrophiler Chemotaxis, scheint bei den in der Folge zu besprechenden nekrotisierenden Arteriitiden noch ein weiterer Mechanismus wichtig zu sein, der bei einigen Syndromen zu den im Vordergrund stehenden granulomatösen Formationen führt [15].
