Die rhythmogene Herzinsuffizienz

Zusammenfassung

Unter dem Begriff „Herzinsuffizienz“ verstehen wir eine sich akut oder chronisch entwickelnde Pumpfunktionsstörung des Herzens, die zu einer Verminderung des Herzzeitvolumens führen kann, welches den Bedürfnissen der Kreislaufperipherie nicht mehr ausreichend angepaßt ist (Vorwärtsversagen) und/oder zu einer Stauung vor dem rechten bzw. linken Ventrikel (Rückwärtsversagen) führt. Klassifiziert nach herzmuskelmechanischen Gesichtspunkten stellen Vorlast, Nachlast, Kontraktilität und Herzfrequenz die wesentlichen Determinanten der Herzauswurfleistung dar. Liegen pathogenetisch einer Herzinsuffizienz Veränderungen der Frequenz der Kontraktion im Gefolge bradykarder oder tachykarder Rhythmusstörungen zugrunde (s. Riecker et al. 1978), sprechen wir von einer rhythmogenen Herzinsuffizienz.
