Zusammenfassung
Das kleinzellige Bronchialkarzinom wird den übrigen histologischen Entitäten maligner Bronchialtumoren wegen verschiedener tumorbiologischer Besonderheiten gegenübergestellt: Es ist gekennzeichnet durch eine rapide Proliferation mit entsprechender kurzer Tumorverdopplungszeit [1], einen hohen Markierungsindex [2] und eine große Wachstumsfraktion [3]. Diese Eigenschaften sind für die bekannte Chemotherapie- und Radiotherapiesensitivität mitverantwortlich. Pathologisch-anatomisch sind kleinzellige Bronchialkarzinome durch stromaarme, dichtgepackte Infiltrate aus fusiformen, polygonalen oder lymphozytenähnlichen („oat cell“) Zellen charakterisiert, welche sich bevorzugt entlang der submukösen Lymphwege ausbreiten und eine diffuse Schwellung der Mukosa bewirken. Die Zellen des kleinzelligen Bronchialkarzinoms entstammen mit großer Wahrscheinlichkeit dem APUD-System („amine precursor uptake and decarboxylation“), welches seinerseits vom Neuralrohr, der Neuralleiste und dem primitiven Entoderm abgeleitet und dadurch in den verschiedenen Organsystemen ausgeprägt ist: innersekretorische Drüsen, spezialisierte Nervengewebe, Urogenitaltrakt, Gastrointestinaltrakt und Respirationstrakt [4]. Elektronenmikroskopisch finden sich in den Tumorzellen charakteristische (wahrscheinlich neurosekretorische) Granula [5]. Kürzlich wurde ein besonderes zytogenetisches Merkmal in praktisch allen Metaphasen von 12 kultivierten Zellinien menschlichen kleinzelligen Bronchialkarzinoms gefunden: es handelt sich um die Deletion 3P (14–23), eine Verkürzung des dritten Chromosoms im Bereich des kurzen Armes, welche bisher in Bronchialkarzinom-Zellinien nicht-kleinzelligen Typs nicht nachweisbar war und welche sich auch bei kultivierten Lymphoblasten der betreffenden Patienten nicht nachweisen ließ [6].
