Zusammenfassung
Die Pathobiochemie, Diagnostik und Therapie der Fettleber - auch als „Leberzellverfettung, Fettinfiltration der Leber“ oder „Steatosis hepatis“ bezeichnet — steht nach wie vor im Mittelpunkt des ärztlichen Interesses. Dies mag einmal an der Häufigkeit ihres Vorkommens liegen, zum anderen an der Tatsache, daß das Symptom Fettleber nicht selten ein Alarmsignal für andere, meist ernstere Krankheiten darstellt. In diesem Zusammenhang ist auf das erstmals von REYE und Mitarbeitern (25) beschriebene, gleichzeitige Vorkommen von Fettleber und Encephalopathie bei Kindern hinzuweisen. Die Pathobiochemie dieses bedrohlichen, im Gefolge von Viruserkrankungen auftretenden Krankheitsbildes ist bis jetzt noch ungeklärt. Neuere morphologische und histochemische Befunde von BOVE und Mitarbeitern (5) deuten auf eine Schädigung der Mitochondrien, eine Störung des Tricarbonsäure-Cyclus und eine mangelnde Ammoniak-Entgiftung in der Leber hin. Es erscheint demnach berechtigt, sich u.a. mit dem Fettstoffwechsel der Leber und seinen Beziehungen zu cerebralen Funktionen zu beschäftigen. Wir werden im folgenden sehen, welche tierexperimentellen Modelle heute für die pathobiochemische Forschung zur Verfügung stehen. Zuvor sollen jedoch einleitend die Definition, Häufigkeit und Ätiologie der menschlichen Fettleber in der hier gebotenen Kürze dargestellt werden.
