Zusammenfassung

Für die Aufgabe der Lunge — ausreichende Sauerstoffaufnahme und Kohlendioxidabgabe unter Wahrung des Säure-Basen-Status — sind nicht nur die absoluten Größen der bekannten Faktoren Perfusion, Ventilation und Diffusion von Bedeutung, sondern auch ihre regionale Verteilung, die Distribution. Bereits beim Gesunden sind Ventilation, Perfusion und Diffusion ungleichmäßig auf die verschiedenen Lungenabschnitte verteilt. So sind bei aufrechtem Oberkörper unter dem Einfluß der Schwerkraft die Alveolen in den basalen Lungenabschnitten kleiner als in den oberen und daher einer stärkeren Ausdehnung fähig. Sie leisten bei normaler Atmung einen größeren Beitrag zum Gasaustausch. Mit zunehmender Atemstromstärke wirken sich zusätzlich die Atemwegwiderstände auf die Verteilung aus. Eine weitere Einflußgröße ist die Atemtiefe (Siegel und Potchen, 1973).
