Zusammenfassung

Die Fortschritte der kardiologischen Diagnostik und Therapie haben auch das Interesse der Klinik an Erfassung und Behandlung von Herz- und Perikardtumoren verstärkt. So ist es zu erklären, daß im letzten Jahrzehnt mehr Herztumoren beschrieben wurden, als in 100 Jahren zuvor. Vor allem mehren sich die Darstellungen von Fällen, in denen eine operative Behandlung möglich war. (Übersichten bei Bailey u. Mitarb., 1959; Castaneda u. Varco, 1968.) Neben elektrokardiographischen Befunden haben röntgenologische Untersuchungsverfahren, in erster Linie die Angiokardiographie, einen wesentlichen Anteil an der präoperativen Diagnostik (Bahnson u. Newman, 1953; Bayer u. Mitarb., 1954; Goldberg u. Steinberg, 1955; Dejdar u. Mitarb., 1959; Steinberg u. Mitarb., 1964; Steiner, 1968). Dieses Verfahren hat besondere diagnostische Bedeutung bei allen intrakavitären, raumfordernden Tumoren, es ist aber auch zur Abgrenzung von Perikardtumoren oder parakardialen Blastomen von Bedeutung, bei denen häufig abnorme Randkonturen und mehr oder weniger bizarre Begrenzungen des Herzschattens erste Hinweise dafür sind, daß ein Tumor, von Herz oder Perikard ausgehend, bestehen kann. Die Diagnose Herztumor läßt sich häufig nur als Vermutungsdiagnose stellen (Mark, 1970); selten ist darüber hinaus eine Aussage über die Natur des Tumors möglich, die für die Prognose und die Operationsindikation von wesentlicher Bedeutung ist.
