Zusammenfassung
Die Lichtkoagulation mit dem Gerät von Zeiss-Oberkochen nach MeyerSehwickerath hat heute weite Verbreitung gefunden. Es haften ihr jedoch gewisse Mängel an, die sich je nach der Lage des Falles mehr oder weniger stark bemerkbar machen:

a)
Bei stark peripherer Lage der Läsionen verschlechtert sich die Bildqualität recht merklich, was sich einerseits in Unsicherheit bei der Beobachtung, andererseits in einer Vergrößerung und Verzerrung des Zielflecks bemerkbar macht. Diese Schwierigkeiten werden noch dadurch verschärft, daß das Beleuchtungsbündel durch die Siebblende in sechs Teilbündel zerlegt wird, die sich bei Aberration des optischen Systems nicht genau superponieren und so die Homogenität des Feldes beeinträchtigen.

b)
Das größtmögliche Sucherfeld hat einen Durchmesser von 6°, was für ein rasches Auffinden der gesuchten Objekte eher etwas knapp ist.

c)
Bei nicht emmetropen Augen bzw. peripherer Lage der Läsionen müssen Kontaktlinsen benutzt werden. Bei Eindellung oder anderweitig bedingter Unebenheit des Augenhintergrundes wird die Fokussierung schwierig.

d)
Verschiedene Faktoren tragen dazu bei, daß die Sicht während der Abgabe des Lichtpulses schlechter ist als während der Beobachtungsphase. Dies wirkt sich direkt darin aus, daß häufig überdosiert, d. h. mehr Energie für ein Koagulat aufgewendet wird, als für den beabsichtigten Zweck notwendig wäre.

e)
Mit dieser Methode scheint in einigen Prozent der behandelten Fälle unvermeidbar eine postkoagulative Maculopathie (Sternfaltensyndrom) einherzugehen, deren Häufigkeit zwischen etwa 1 bis 4% Hegen dürfte.
