Zusammenfassung
Die Einführung der perkutanen transluminalen Koronarangioplastie (PTCA) durch Grüntzig im Jahre 1977 eröffnete eine neue therapeutische Strategic zur minimalinvasiven Behandlung koronarkranker Patienten [14] und markierte den Beginn der interventionellen Kardiologie. Eine dauerhafte Wiederherstellung des koronaren Blutflusses nach konventioneller Ballonangioplastie war jedoch eingeschränkt durch elastische Rückstellkrafte am gedehnten Koronararteriensegment, durch akute Gefäßverschliisse aufgrund okklusiver Intimadissektionen und Thrombosen sowie durch spätere Wiederverengungen des dilatierten Gefäßsegments infolge einer reaktiven Intimahyperplasie [5, 7, 25, 36, 44, 53]. Die Entwicklung alternativer interventioneller Techniken, bei der ablative und dilatierende Verfahren zur Beseitigung stenosierender Atherome kombiniert wurden [27, 43, 48, 61], ergab gegenüber der konventionellen Ballonangioplastie bislang allenfalls partielle Verbesserungen, die sich in erster Linie auf eine Steigerung des interventionellen Primärerfolges beschränkten.
