Zusammenfassung
Zahlreiche Verlautbarungen in der Laienpresse sowie in Rundfunk-und Fernsehsendungen haben das Interesse an Hefen im Magen-Darm-Trakt in der breiten Öffentlichkeit geweckt. Die Besiedlung des Gastrointestinaltraktes mit Candidaspezies, besonders Candida albicans, wird für eine Vielzahl unspezifischer Symptome verantwortlich gemacht (Tabelle 1). In den auf dem Markt befindlichen Büchern von ärztlichen und nichtärztlichen Vertretern der Antihefepilzbewegung finden sich stets sog. „Checklisten“ mit Symptomen, die Hinweis auf eine Pilzinfektion im Darm sein könnten. Der Katalog an verdächtigen Symptomen ist dabei so breit gefaßt, daß es kaum jemanden geben wird, der nicht mindestens eine der Fragen mit „ja“ beantworten würde. Dem Leser dieser Bücher wird dann empfohlen, den Arzt seines Vertrauens zu bitten, die notwendige Diagnostik und ggf. eine antimykotische Therapie einzuleiten. Entsprechend bieten eine ständig wachsende Zahl mykologischer und bakteriologischer Labors ihre Dienste für die Stuhldiagnostik auf Hefen an. Verfechter der Antihefepilzbewegung vertreten die Ansicht, daß jeder Nachweis von Candida albicans in der Mundhöhle oder im Stuhl behandlungsbedürftig sei. Ein positiver Nachweis impliziert für sie stets eine mehrwöchige medikamentöse Behandlung mit Antimykotika, in der Regel Nystatin, eine mehrmonatige sog. „Antihefepilzdiät“ und die Einahme von Präparaten zum Aufbau der Darmflora. Der Diät kommt bei der erfolgreichen „Bekämpfung“ der Hefen im Darm eine große Rolle zu, „Diätfehler“ werden als häufigste Ursache für Rezidive angesehen.
