Zusammenfassung
Das Hypopharynx-(Zenker-)divertikel ist eine hernienartige Aussackung von Mukosa und Submukosa an der unteren Hypopharynxhinterwand. Die Durchtrittstelle liegt im sogenannten Killianschen Dreieck, das von der Pars obliqua und der Pars fiindiformis des Musculus cricopharygeus gebildet wird (4). Als Therapie der Wahl galt lange Zeit die einzeitige Abtragung des Divertikelsackes von außen mit einer zusätzlichen Myotomie des Schleudermuskels (Pars fundiformis des M. cricopharyngeus). 1917 beschrieb MOSHER erstmals eine endoskopische Behandlung des Zenker-Divertikel, indem er die Trennwand zwischen Divertikel und Oesophagus mit einem überlangen Messer durchschnitt (10). 1981 führte VAN OVERBEEK die Schwellendurchtrennung erstmals mit einem C02-Laser unter mikroskopischer Kontrolle durch (11).
