Zusammenfassung
Die Behandlung gastrointestinaler Karzinome basiert auf der chirurgischen Tumorentfernung, wobei die Heilungsraten unverändert unbefriedigend sind, da eine Dissemination von Tumorzellen zum OP-Zeitpunkt auch bei frühen Tumorstadien bereits häufig eingetreten ist [1], Die Elimination dieser Zellen und potentiell bereits bestehender Mikrometastasen ist das Ziel adjuvanter Therapiekonzepte. Hierbei bieten monoklonale Antikörper (mAk) die Aussicht auf eine Behandlungsform, die eine hohe Spezifität mit einer geringen Toxizität verbindet und von der ein therapeutischer Effekt in der Behandlung colorektaler Karzinome im Dukes C Stadium bereits nachgewiesen werden konnte [7]. Die Elimination von Tumorzellen durch mAk erfolgt über die Induktion einer zellulären Abwehr (ADCC = antibody-dependent cellular cytotoxicity), sowie einer Aktivierung von Komplement (CDC = complement-dependent cytotoxicity). Eine Komplementaktivierung kann jedoch durch membranständige Resistenzfaktoren (CD46, CD55 und CD59) blockiert werden, wodurch nicht nur die Zellyse verhindert, sondern auch die Bildung wichtiger Mediatoren der zellulären Abwehr (u.a. C3a und C5a, C3b, C3d) unterbunden werden kann [4].
