Zusammenfassung
Das Lynch-Syndrom (hereditäres non-Polyposis kolorektales Karzinom, HNPCC) ist ein familiäres Krebsprädispositionssyndrom, das mit einer erhöhten Inzidenz von Adenokarzinomen (kolorektales, Endometrium-, Magenkarzinom u.a.) einhergeht. Ursache der Erkrankung sind Keimbahnmutationen in Genen (hMSH2, hMLH1, hPMS1, hPMS2), deren Expression für eine korrekte Reparatur von DNA-Replikationsfehlern unerläßlich ist. Infolgedessen treten in reparaturdefizienten Tumorzellen multiple DNA-Replikationsfehler auf, und zwar sowohl in kodierenden (u.a. TGFß-Rezeptor Typ II-, p53-, APC-Gen) als auch in nicht-kodierenden Abschnitten. Dies läßt sich am einfachsten im Bereich von Mikrosatelliten-DNA — kurze repetitive Nukleotidsequenzen — als Längenvariationen (sog. Mikrosatelliteninstabilität, RER+) nachweisen. Bislang war eine Diagnose des Lynch-Syndroms ausschließlich aufgrund klinischer Kriterien möglich. Ziel unserer Arbeit ist die Etablierung eines Verfahrens zur molekularbiologischen Diagnose des Lynch-Syndroms, das eine Erfassung von Anlageträgern vor Manifestation der Erkrankung und gegebenenfalls eine prophylaktische Therapie ermöglichen soll.
