Zusammenfassung
Die Literaturangaben über die Befallshäufigkeit des oberen Sprunggelenkes bei der chronischen Polyarthritis im Erwachsenenalter differieren sehr stark: von gut 10% bis über 50% [6, 20, 40, 46, 47]. Bei der juvenilen chronischen Polyarthritis mit ihren verschiedenen Formen stimmen dagegen die Zählungen weitgehend überein. Hier liegt das Sprunggelenk nach dem Kniegelenk mit über 80% Befallshäufigkeit an 2. Stelle [1, 4, 23, 40, 42]. Unter pathomechanischen Gesichtspunkten muß der rheumatische Fuß als funktionelle Einheit betrachtet werden [40, 41, 46]. Deformierungen und Funktionsstörungen im Rückfußberreich wirken sich auch zwangsläufig auf den Vorfuß aus [41]. Als Beispiel von besonderer Bedeutung sei die tibiale Fußsäule genannt. Schmerzhafte entzündliche Veränderungen sowohl der Zehen- als auch der Fußwurzelgelenke führen hier gleichermaßen zu einer Elevation des Metatarsale [46]. Die Fixierung dieser anfänglichen Schon- und Fehlhaltung führt zu einer Abflachung des Längsgewölbes sowie zu einer Abduktion und supinatorischen Aufdrehung des Mittel- und Vorfußes. Wenn nach Abklingen der akuten schmerzhaften Entzündung entweder infolge abklingender Beschwerden oder aber durch Muskelschwäche der Fuß wieder plantigrad belastet wird, wird ein Pes planovalgus manifest: die häufigste rheumatische Rückfußveränderung. Außerdem bedingt der „Metatarsus primus elevatus“auch ein Verlust des Quergewölbes — einer der Hauptgründe für die charakteristischen Zehendeformierungen, die sinngemäß spreizfußbedingten Veränderungen entsprechen, diese aber an Ausprägung oft weit übertreffen.
