Laparoskopie und laparoskopische Kontaktultrasonographie bei Erkrankungen der Leber, des Gallengangsystems und der Bauchspeicheldrüse

Zusammenfassung

Die diagnostische Laparoskopie ermöglicht im Hinblick auf die Behandlung akuter und chronischer intraabdomineller Erkrankungen höchst relevante klinische Untersuchungsergebnisse, besonders hilfreich ist sie jedoch bei Erkrankungen der Leber, des Gallengangsystems und der Bauchspeicheldrüse. Bei malignen intraabdominellen Erkrankungen [1–9] ist das Verfahren nicht nur zur Artdiagnose, sondern auch zur Beurteilung des Tumorstadiums so eindeutig von Nutzen, daß sich die Frage stellt, weshalb es nicht schon früher als präoperative Untersuchung routinemäßig angewendet wurde; immerhin hat sich die Situation im Lauf der letzten Jahre drastisch verändert. Anstelle der wiederholten Anwendung teurer Verfahren wie der Computertomographie (CT) und der Magnetresonanztomographie (MRT) mit oder ohne perkutane Biopsieentnahme ermöglicht die diagnostische Laparoskopie in den meisten Fällen nicht nur die direkte Inspektion des Tumors und die Entnahme einer zuverlässigen Gewebeprobe, sondern häufig auch die exakte Bestimmung des Krankheitsstadiums und liefert Informationen über die Ausbreitung und Resezierbarkeit des Primärtumors.
