Zusammenfassung
Heparin und Hirudin beeinflussen den Gerinnungsablauf auf unterschiedliche Weise: Heparin benötigt Antithrombin III, um effektiv die Gerinnung zu hemmen [1, 2]. Heparin plus Antithrombin III inaktivieren nicht nur Thrombin, sondern auch andere Gerinnungsfaktoren, wie zum Beispiel Faktor Xa [3, 4]. Hirudin ist Cofaktor-unabhängig und bildet einen praktisch unlöslichen nicht kovalenten Komplex mit Thrombin [5]. Heparin und Hirudin beeinflussen das Gerinnungssystem zusätzlich durch Hemmung der Thrombin abhängigen Feedback Aktivierung von Faktor V und Faktor VIII [6, 7]. Die Bedeutung dieser Mechanismen für die Wirkungsweisen von Heparin und Hirudin ist nicht restlos geklärt. Die direkte Inaktivierung von Thrombin durch Antithrombin III oder Hirudin kann durch die Messung von Thrombin-Antithrombin-III-Komplex (TAT) und Throm-bin-Hirudin-Komplex (THC) bestimmt werden. Wir haben daher TAT, THC, Prothrombinfragmente 1 + 2 (F 1 + 2) und freies Thrombin in plättchenarmem Zitratplasma nach intrinsischer Aktivierung mit oder ohne Zusatz von Heparin oder Hirudin gemessen.
