Zusammenfassung
Als Folge immer differenzierterer Therapiemodalitäten haben Streßulkusblutungen bei chirurgischen Hochrisikopatienten beträchtlich abgenommen. Gleichzeitig kam unter einem großzügigen Einsatz von H2-Blockern der Verdacht einer erhöhten nosokomialen Pneumonieinzidenz mit deutlich erhöhter Letalität auf, hervorgerufen durch vermehrte gastrale Keimbesiedlung nach pH-Anhebung des Magensaftes und gastroösophagealen Reflux mit kontaminiertem Magensekret. Allerdings zeigen neuere placebokontrollierte Studien [3, 6] eine höhere Blutungsrate als Prophylaxegruppen, eine Studie [5] findet bei Einsatz eines speziellen subglottischen Spültubus weniger Pneumonien. Darf in Kenntnis dieser Problematik heutzutage eine generelle Ulkusprophylaxe bei chirurgischen Intensivpatienten überhaupt noch erfolgen und welche Patienten profitieren von welcher Prophylaxe?
