Prinzipien der Kombination von Radiotherapie und Chemotherapie

Zusammenfassung

Die Kombination von Radiotherapie und Chemotherapie hat in der Therapie von Lymphomen und soliden Tumoren große Bedeutung erlangt und wird im Rahmen kurativer Konzepte der Kinder- und Erwachsenenonkologie durchgeführt. Grundsätzlich gilt für solide Malignome des Erwachsenen, daß eine alleinige Strahlentherapie nicht nur Tumoren mikroskopischer, sondern auch makroskopischer Größe dauerhaft vernichten kann. Hingegen beherrscht — mit Ausnahme der Karzinome des Hodens — die Chemotherapie solide Tumoren nur, wenn eine mikroskopische Größe nicht überschritten ist. In einer Kombinationsbehandlung, in der beide Therapiemodalitäten am selben makroskopischen Karzinom oder Sarkom zytotoxisch wirken, ist also die Rate der durch Chemotherapie abgetöteten Tumorzellen im Vergleich zur Strahlentherapie um Größenordnungen kleiner. Dieser Aspekt sollte auch bei der Durchführung neoadjuvanter Therapiekonzepte (präoperative Kombination von Radiotherapie und Chemotherapie) berücksichtigt werden.
