Zusammenfassung

Das Prostatakarzinom ist zum häufigsten malignen Tumor des älteren Mannes geworden. Nur vereinzelt wird es vor dem 50. Lebensjahr beobachtet, danach steigt die Häufigkeit rasch an. In den europäischen Ländern und den USA steht dieser Tumor in der Todesursachenstatistik der Männer an zweiter Stelle nach den Lungentumoren. Jährlich sterben daran in Deutschland etwa 12 000 Männer. In den USA hat die Inzidenz zwischen 1970 und 1985 (56 und 75 Neuerkrankungen pro 100 000 weiße Einwohner) beträchtlich zugenommen; dies wird auf die verbesserte Früherkennung zurückgeführt (Devesa et al. 1987). In Deutschland ist die Inzidenz zwischen 1975 und 1988 gleich geblieben, im Rahmen der gesetzlichen Früherkennung wurden 85 Karzinome bei 100 000 Untersuchten entdeckt (Flatten 1991). Diese Zahl ist jedoch nicht mit der Inzidenz gleichzusetzen, die mit 66/100 000 angenommen wird. Die Inzidenz ist bei schwarzen Männern in den USA mit 95 pro 100 000 deutlich höher als bei der weißen Bevölkerung, besonders niedrig ist sie in Japan und China (Doll 1978). Wenn Japaner in die USA einwandern, nimmt auch bei ihnen die Inzidenzrate langsam zu (Haenszel u. Kurihara 1968).
