Zusammenfassung
Verbesserte diagnostische Verfahren in der Dermatoonkologie sind wünschenswert im Hinblick auf eine präzisere Artdiagnose, Information über die Tumorausbreitung und zum Nachweis von minimaler Resttumormasse nach klinisch scheinbar kompletter Re-mission. Verfahren, die zum Teil derzeit noch als „experimentell“bezeichnet werden müssen, werden in der Zukunft aber möglicherweise von praktisch-klinischer Relevanz sein. Die Positronenemissionstomographie hat eine Sensitivität von über 90%. Die Durchführung der Magnetresonanz bei Hauttumoren stellt besondere, technisch noch unzureichend gelöste Anforderungen an den Magnetkopf. Die Immunszintigraphie mit markierten LAK-Zellen hat in ihrer Aussagekraft bisher enttäuscht. Im Mikrobereich können morphometrische, Ultraschall- und lasermikroskopische Methoden weiter ausgebaut werden. Die Epilumineszenz ist eine wertvolle inzwischen auch in der Praxis etablierte Technik. Oberflächenstrukturanalytische Untersuchungen wer-den ihren primären Wert sicherlich nicht im Rahmen der Dermatoonkologie entfalten. Die Immunhistochemie mit monoklonalen Antikörpern ist ebenso wie die Genotypisierung ein inzwischen etabliertes Verfahren, das zunehmend ausgebaut wird und möglicherweise auch in der Frühdiagnose und der Detektion von „minimal residual disease“eine wichtige Rolle spielen wird. Der Stellenwert serologischer Untersuchungen beim Melanom (5-S-Cy- steinyldopa) und beim malignen Lymphom (Adenosindeaminase, Interleukin-2-Rezeptoren) ist noch nicht abzuschätzen. Neue Analysen von Prognoseparametern beim malignen Melanom haben zu Empfehlungen von Tumordickengrenzen bei 1 und 2 mm geführt. Bei den kutanen T-Zell-Lym phomen ist die Bestimmung der „Tumormasse“(Tumor-Burden-Index) von wesentlicher prognostischer Bedeutung.
