Zusammenfassung
Empfänger von Nierentransplantaten (NTX) sind prädisponiert für Ulkuserkrankungen im oberen Gastrointestinaltrakt aufgrund mehrerer Risikofaktoren. Vor NTX kann die Urämie zu Hypergastrinämie [1] und sekundärem Hyperparathyreoidismus führen. Nach NTX können die chronische Immunsuppression mit verminderter Resistenz gegenüber bakteriellen, fungalen oder viralen Krankheitserregern sowie der tertiäre Hyperparathyreoidismus das Auftreten eines Ulkus bedingen. Daher wird mancherorts die routinemäßige prophylaktische Ulkusoperation vor NTX bei entsprechend veranlagten Patienten für notwendig erachtet [2]. Fortschritte auf den Gebieten der Immunsuppression und der Behandlung von Infektionen im letzten Jahrzehnt [3, 4] veranlaßten uns, die Inzidenz des oberen gastrointestinalen Ulkus (Ösophagus, Magen, Duodenum) und seiner Komplikationen nach NTX im eigenen Krankengut seit Einführung des Ciclosporin A (CSA) zu untersuchen und die Frage nach der Notwendigkeit prophylaktischer Ulkusoperationen vor NTX zu beantworten.
