Neoadjuvante Radiochemotherapie in der Behandlung des Rektumkarzinoms

Zusammenfassung

Mit einem kombinierten präoperativen Radiochemotherapie-Protokoll konnte bei 97 Patienten mit fortgeschrittenen Rektumkarzinomen ein hohes Maß an lokaler Tumorkontrolle erreicht werden. Es resultierte ein signifikanter down-staging-Effekt über alle Tumorstadien. In 11% trat eine Vollremission ein. Trotz initial fortgeschrittener Tumore konnten hohe Resektionsraten (99%) erreicht werden. Die postoperative Komplikationsrate war gegenüber dem Vergleichskollektiv nicht signifikant erhöht. Jedoch ist aufgrund der schlechteren Heilungstendenz im vorbestrahlten Gewebe bei Komplikationen ein aktiv-chirurgisches Vorgehen gefordert.

Summary

Ninety-seven patients with advanced rectal carcinoma (clinical staging III/IV) were preop- eratively treated with radiation therapy (40 Gy) and two cycles of 5–fluorouracil and mitomycin C (RCTH). Overall, 71% of the patients showed a good clinical response, the resection rate was 99%, and a pathologic complete response occurred in 11%. A pair- matching study showed a significant (p<0.05) downstaging in postoperative T stage for patients after RCTH versus patients who underwent operation alone, while the complication rate was comparable.
