„Proximal-gastrale Vagotomie: Laparoskopische Technik und Ergebnisse“

Summary

Despite successful eradication therapy and long-term acid reduction by drugs, 10% of duodenal ulcer patients require operation because of recurrent ulcers. Laparoscopic proximal gastric vagotomy, based on a well-documented, conventional technique, is a valuable therapeutic measure for recurrent duodenal ulcer disease. An operation time of between 2 and 3 h is acceptable for this demanding procedure. In 9 of 22 patients postoperative acid reduction to 30% of the norm has been documented. Convincing is the high patient comfort; therapy must be performed once, and is minimally invasive with excellent cosmetic and functional results. The comparatively low costs are last but not least important arguments.

Zusammenfassung

Trotz erfolgreicher Eradikationstherapie und medikamentöser Säurereduktion verbleibt bei 10% der Ulcus duodeni Patienten wegen rezidivierender Ulcera eine Operationsindikation. Die laparoskopische proximal-gastrale Vagotomie basiert in ihrer Technik auf der konventionellen PGV und hat somit ein bewährtes, standardisiertes und evaluiertes Verfahren der Säurereduktion zur Grundlage. Die Operationstechnik ist anspruchsvoll, die Operationszeit mit 2–3 Stunden akzeptabel. Bei 9 von 22 Patienten konnte bisher postoperativ die Säurereduktion auf 30% der Norm dokumentiert werden. Überzeugend ist die geringe Patientenbelastung, das gute funktionelle und kosmetische Resultat und vor allem das einzeitig kurative Therapiekonzept, welches auch der Kostendiskussion entgegenkommt.
