Summary
The indication for small bowel transplantation is the short bowel syndrome. Due to long-term total parenteral nutrition, the liver is damaged in many cases. Thus, the simultaneous replacement of the liver is sometimes necessary. The special immunological role of the liver, particularly regarding the rejection reaction, raises the question of whether a simultaneous liver graft would have an immunologically beneficial effect on the small bowel when transplanted orthotopically. Therefore we developed an animal model simultaneously transplanting liver and small bowel in the rat. The results indicate that the orthotopic replacement of liver and small bowel with arterialization of the liver graft and portal venous drainage of the small bowel graft into the liver is a reliable standardized technique. The perioperative mortality is similar to the orthotopic rat liver transplantation model (<3%) and the long-term survival rate of a syngeneic recipient is similar to that of recipients of a singular small bowel graft (>90%). Hence, with this new technique, the clinical situation is imitated exactly. Urgent immunological questions can be analyzed in a standardized fashion with this model.
Zusammenfassung
Die kausale Therapie des Kurzdarmsyndroms besteht in einer Dünndarmtransplantation. Aufgrund häufig schwerer Leberschäden, entstanden durch die langzeitige parenterale Ernährung, ist in vielen Fällen die simultane Lebertransplantation indiziert. Da die Leber eine immunologische Sonderrolle einnimmt, insbesondere im Hinblick auf das deutlich reduzierte Abstoßungsverhalten allogener Lebertransplantate, soll der Frage nachgegangen werden, ob eine simultane Leber-Dünndarm-Transplantation immunologisch vorteilhaft ist. Daher wurde ein dem klinischen Einsatz nahestehendes Tiermodell entwickelt, bei dem Leber und Dünndarm simultan orthotop ersetzt werden. Es hat sich gezeigt, daß der simultane Ersatz mit arterialisiertem Lebertransplantat und portal-venös drainiertem Dünndarmtransplantat technisch gut durchführbar und standardisierbar ist. Die Operationsletalität entspricht der der orthotopen Lebertransplantation, die Langzeitüberlebensraten liegen mit über 90% im Bereich der orthotopen Dünndarmtransplantation an der Ratte. Die sich ergebenden immunologischen Fragestellungen sind mit diesem Modell exakt standardisierbar zu untersuchen.
