Der axillo-femorale Bypass zum Extremitätenerhalt bei Hochrisikopatienten – Eine schonende Alternative

Summary

Between 1984 and 1993, 66 patients with chronic arterial occlusive disease were provided with axillofemoral bypass. General complications were found in 35.2% and local complications in 24.4%; 9.1% of the patients died within 30 days of surgery. During the follow-up 30% of the patients died within 3 years after surgery. The 3–year patency for the follow-up patients is 67%. Considering the operative mortality, the long-term results, the patency results, and the high late mortality, independent of the surgical procedure, the extraanatomic bypass is used in revascularizing high-risk patients with rest pain and necrosis or when laparotomy is contraindicated.

Zusammenfassung

Im Zeitraum von 1984 bis 1993 erhielten 66 Patienten eine axillo-fe- morale Prothese. Allgemeine Komplikationen fanden sich bei 35,2%, lokale Komplikationen bei 24,4%. Während die postoperative Letalität bei 9,1% lag, waren nach 3 Jahren bereits über 30% der Patienten verstorben. Die Offenheitsrate für das nachuntersuchte Patientenkollektiv lag nach 3 Jahren bei 67%. Obwohl die Kurz- und Langzeitergebnisse zufriedenstellend sind, bleibt der extraanatomische Bypass, insbesondere im Hinblick auf die Spätletalität, auf Hochrisikopatienten im Stadium III/IV oder bei Kontraindikation gegen eine anatomische Rekonstruktion beschränkt.
