Summary
During the last 20 years, the prognosis for central bile duct carcinomas has improved due to the development of extended surgical procedures. For resectable tumors a 1-, 2-, 3-, and 5-year survival rate of 67%, 39%, 24%, and 13%, respectively, has been reported (Boerman 1990; n = 499). After palliative resections, the survival rates could not be improved either in patients with gallbladder carcinomas or those with central bile duct carcinomas, as shown in several series. Because a planned palliative resection cannot improve the survival rate but involves quite an operative risk, the indication for such an intervention has to be discussed critically until sufficient adjuvant therapy protocols exist.
Zusammenfassung
Während der letzten 20 Jahre hat sich die Prognose des zentralen Gallenwegskarzinoms durch Entwicklung radikaler Operationsverfahren etwas verbessert. Die 1-, 2-, 3- und 5-Jahresüberlebensraten werden bei resektablen Tumoren mit 67%, 30%, 24% und 13% (Boerman 1990, 499 Fälle) angegeben. Für palliative Resektionen konnte durch die alleinige chirurgische Therapie bisher keine Verbesserung der Überlebensrate nachgewiesen werden. Die palliative Resektion hat ebenfalls beim Gallenblasenkarzinom keinen Einfluß auf die Überlebensrate, wie in mehreren Studien gezeigt werden konnte. Da für eine geplante palliative Resektion keine Verbesserung der Überlebensraten nachgewiesen ist und diese ein nicht zu vernachlässigendes Operationsrisiko beinhaltet, kann sie erst dann einen Stellenwert in der onkologischen Chirurgie erhalten, wenn suffiziente adjuvante Therapiemaßnahmen zur Verfügung stehen, die das Risiko für diesen Eingriff rechtfertigen würden.
