Zusammenfassung

Von 218 Tibiaverriegelungsnagelungen konnten 154 klinisch und röntgenologisch (x = 26 Monate) überprüft werden. Operationsindikationen: geschlossene (n = 102) und offene (n = 74) Unterschenkelfrakturen. Pseudarthrosen, Korrektur- Umstellungsosteotomien (n = 28). Mono-Rail-Verfahren (n = 14). Operationstechnische Probleme: Malrepositionen (n = 22), falsche Nageleintrittsstelle (n = 15), inadäquate Nagellänge (n = 12), offene Reposition bei G-Frakturen (n = 7), Bolzenfehllage (n = 5), iatrogene Frakturen (n = 4), Bohrerbruch (n = 4). Implantatversagen: Nagelbrüche: AO-UTN (Mono-Rail: n = 2; Pseudarthrose: n = 1). RT-Delta-Nagel (Mono-Rail: n = 1; „Delayed Union“: n = 1). Verriegelungsbolzenbrüche: AO-UTN: 12% der Nägel. R-T-Delta: 5% der Nägel.

Intraoperative Problems and Implant Failure with Unrearned Interlocking Tibial Nails

Summary

Between December 1988 and December 1993, 218 unreamed interlocking tibial nailings were carried out. Clinical and radiographic examination (x = 26 months) was possible in 154 cases. Indications were closed (n = 102) and open (n = 74) fractures, delayed unions, nonunions, and malunions (n = 28), and the monorail procedure (n = 14). Intraoperative complications included malreduction (n = 22), false starting point (n = 15), inadequate nail length (n = 12), open reduction in closed fractures (n = 7), misplacement of locking bolts (n = 5), iatrogenic fractures (n = 5), and broken drill parts (n = 4). Implant failure due to nail breakage occurred in AO-UTN (monorail, n = 2; nonunion, n = 1) and Russell-Taylor-Delta groups (monorail, n = 2; delayed union, n = 1). Breakage of screws occurred in 12% of all nails in the AO-UTN group and in 5% of nails in the R-T-Delta group.
