Chirurgische Therapie der extrahepatischen Gallengangszysten

Summary

A total of 17 patients with extrahepatic biliary cysts (11 type I, three type II, and three type III, according to the classification by Todani) are reviewed with emphasis on treatment and long-term results after a follow-up of 6 months to 12 years. Radical excision of the dilated bile duct and reconstruction by Roux-en-Y hepaticojejunostomy were performed in 13 cases (nine type I, three type II, and one type III), combined in three cases with an antireflux valve. All 13 patients who underwent surgery remained in good health for 6 months to 12 years. These results confirm the need for complete early excision of type I and II cysts at all ages before severe complications can occur. Endoscopic treatment of type III choledochocele should be limited to the management of smaller lesions.

Zusammenfassung

17 Patienten mit extrahepatischen Gallengangszysten (11 × Typ I = Choledochuszyste, 3 × Typ II = Choledochusdivertikel und 3 × Typ III = Choledochozele) werden hinsichtlich der durchgeführten Therapie und der Spätergebnisse (Followup 6 Monate – 12 Jahre) vorgestellt. In 13 Fällen (9 × Typ I, 3 × Typ II und 1 × Typ III) wurde die Zyste radikal exzidiert und eine Hepatico-Jejunostomie nach Roux, die in 3 Fällen mit einer Antireflux-Klappenbildung kombiniert wurde angelegt. Alle radikal operierten und endoskopisch papillotomierten Patienten waren im o.g. Beobachtungszeitraum beschwerdefrei. Unsere Erfahrungen bekräftigen, angesichts des hohen Entartungsrisikos und der zahlreichen Komplikationen nach enteraler Drainage der Zyste, die Notwendigkeit der dringlichen radikalen Exzision der Typ I- und Typ II-Zysten, um den Patienten einen beschwerdereichen Weg zu ersparen. Bei der Choledochozele richtet sich das Vorgehen nach dem anatomischen Bild und der Größe der Zyste.
