Bronchopleural Fistulas: Management of Fistulas with Specially Prepared Spongiosa in Combination with Fibrin Sealant

Abstract

Bronchopleural fistulas are a severe complication occurring mostly after central resections. Nearly all cases are observed following pneumonectomies. Fistulas were observed in 3.7% of all pulmonary resections. In most cases the underlying disease is bronchial carcinoma. The most elegant and easiest way of closing a fistula is endoscopic sealing. Fistulas smaller than 3 mm can be closed with fibrin sealant alone. Using this technique the endoscopic operation must be repeated at least twice. Fistulas larger than 3 mm can be closed only with a combination of specially prepared spongiosa and fibrin sealant. We have used this technique over the past 10 years in 33 patients. About 25% of the fistulas were treated successfully.
